ME Awareness Conference attendance form

ME/CFS/FMS Conference
Wednesday May 19th 2004, Queens Hotel Lockerbie.

The morning session will be geared more to sufferers and carers, whilst the afternoon is for the medical profession, having said that it is hoped that later on in the day there will be a coming together of all the speakers to take part in a question time before the event is brought to a conclusion.
You are welcome to attend morning, afternoon or any part of either session, if that is all you can manage. There will be no charge to attend.

Because this event has been discussed at the Cross Party Group on ME at the Scottish Parliament, a lot of interest has been generated. We have had enquiries from Edinburgh, Glasgow and Ayrshire from people interested in attending. It is vital that you fill in the form below or phone or e-mail indicating if you are going to attend as we may have to limit numbers.

Due lack of forward booking the bus has been cancelled, however we can help you to arrange transport. Use the blank boxes to tell us where you are and we will get back to you.
